The AAPACN Clinical Leader of the Year Award
The AAPACN Clinical Leader of the Year Award is presented to a clinical leader who embodies the four values of AAPACN – inspiration, influence, integrity, and innovation.
AAPACN is proud to celebrate the work of long-term and post-acute care professionals who embody the vision and values of AAPACN. The winner of the AAPACN Clinical Leader of the Year Award will be honored at the AAPACN 2025 Conference, April 22-25, 2025, in Louisville, KY.
Requirements:
- Must be a current AAPACN member in good standing.
- May not have won this award or the DON of the Year Award in the past.
- Must be a resident of the United States.
- Applications must be submitted by midnight MT Nov. 8, 2024.
Award Inclusions:
- The recipient of this award will receive a complimentary registration to the AAPACN 2025 Conference, three-night hotel stay at the conference hotel, and a $500 travel stipend.

1. Give us examples of how you (or the nominee) embody the four values of AAPACN – inspiration, influence, integrity, and innovation.
2. Describe the positive impact you (or the nominee) have had on your facility, residents, or staff. - We highly encourage the submission of recommendations, particularly for those submitting their own nominations. Please forward this recommendation form to your supervisor, administrator, mentor, or peers.
- Include name and facility in the requested fields, but please omit from the other questions, as the application will be made anonymous. Please use “the candidate” and “at our facility” when describing the candidate and their work.
