When it comes to discharge planning, most long-term care professionals can recite some of the pieces—discharge planning starts on admission, requires the entire interdisciplinary team, and doesn’t stop on discharge. But how does a team use all these pieces to meet both the federal regulations and the RAI User’s Manual requirements for discharge planning? During this 60-minute webinar, AAPACN Curriculum Development Specialists, Denise Winzeler, RN, BSN, LNHA, DNS-MT, QCP-MT, and Jessie McGill, RN, BSN, RAC-MT, RAC-MTA, will provide a realistic approach to putting together a discharge plan that meets all of the requirements and helps to achieve successful discharge goals.
After participating in this session, learners will be able to:
• Recognize the federal requirements for F-tag F660, Discharge Planning Process
• Understand how MDS data is used to develop a comprehensive care plan that addresses the discharge plan
• Identify facility systems and processes that support an interdisciplinary approach to discharge planning
