Prep and Planning: Social Determinants of Health | Jessie McGill, RN, BSN, RAC-MT, RAC-MTA, AAPACN
Several new items coming to the MDS Oct. 1, 2023, are also social determinants of health (SDOH) that require care planning considerations and impact discharge planning. During this breakout session, the speaker will help prepare facility staff to ask residents about these new items, document responses, and effectively use the information in care plans and discharge planning.
Section GG – What’s New and How to Implement Workflow Changes | Joel Van Eaton, BSN, RN, RAC-CT, RAC-CTA, RAC-MT, Broad River Rehab
Section GG has replaced section G and has been revised to become the standardized set of data elements for determining resident functional abilities, reimbursement, and a potential future universal post-acute prospective payment system (PPS). These revisions and this standardization will have a wide-ranging impact on many areas including daily operations, patient outcomes, reimbursement, care planning, and quality measurement. During this session, you will gain a broader understanding of the impact that the revision and standardization of section GG will have on skilled nursing facility (SNF) operations, how facilities can implement consistent coding processes, and what will be necessary for accurate completion.
Prepare for MDS Changes: Resident Voice, Scripted Interviews, and Care Planning | Jennifer LaBay, RN, RAC-MT, RAC-MTA, QCP, CRC, AAPACN
MDS updates in sections B, D, F, J and Q effective Oct. 1, 2023, enhance the resident interviews that impact care planning considerations and discharge planning. Some of these updates will change current workflow practices and may also impact Patient-Driven Payment Model (PDPM) reimbursement. During this breakout session, the speaker will help prepare facility staff on how to use the resident’s voice more effectively in the development of person-centered care plans to improve quality of life, enhance discharge planning, and mitigate potential impact to reimbursement.

This breakout session will provide an in-depth discussion regarding the changes to section K, N, and O, as well as review clarifications to pressure ulcer coding in section M that will be effective Oct. 1. Attendees will understand what operational changes may need to be made to capture the additional data that will be required on the MDS item sets beginning this fall.
